Your install keeps running as-is — you just stop receiving updates, lab-API fixes and security patches. To re-subscribe later you pay the missed years plus the current year (prorated catch-up). Most clinics stay current because lab APIs and security patches flow through maintenance.

No — that's what the Nexus In a Box appliance ($2,995) is for. It arrives pre-built, pre-configured and plug-in ready; you connect it to your network and log in. If you have IT, you can self-host on your own hardware or cloud account instead.

On your own appliance or server — never with us. You're the data custodian, which is the simplest PIPEDA / PHIPA posture. Nexus is the software vendor, not a data processor. Encrypted health-card data, full audit log and antivirus scanning are built in.
